About Tanya
As a young girl, I was drawn to spending time with older adults who lived in retirement communities and senior housing and expressed at the age of seven, I want to care for older adults when I grow up. It wasn't until I started to reflect on my childhood milestones, needing to wear glasses and hearing aids at ages 8, then 10-12, that I felt more in common with senior adults than kids my age. I also often felt like an outsider, as others frequently reminded me, until I was volunteering in senior facilities where I felt more at home. These formative experiences shaped me into an individual who now inspires my patients to believe in their capacity to forge new possibilities. I identify with roles of Nurse Practitioner, Empath, Truth Seeker, Teller and Personal Consultant and love pulling from each of them when appropriate to craft personalized experiences for caregivers.
I have consistently demonstrated advanced clinical expertise and strong decision-making skills. Throughout my career, I have been dedicated to diagnosing and treating a wide range of conditions, including dementia, hypertension, diabetes, chronic kidney disease, various cancers, chronic heart and lung diseases, pain management, sleep disorders, and other chronic health issues.

My approach places a strong emphasis on listening to your concerns, providing insightful guidance, and collaborating with you to make well-informed decisions regarding your mind, body, spirit, and soul. When working with me, you can expect a compassionate, insightful, and safe experience, knowing that you are in the hands of an experienced and knowledgeable professional.
This combination equips me to adeptly navigate the myriad of intricate challenges in the realm of geriatric and palliative care.
Before the challenges of progressive chronic illnesses lead to distressing hospitalizations, medical appointments, and treatments misaligned with your loved one's values, I am here to guide you through the process of coping with diagnoses, making informed treatment choices, and being a compassionate presence as your loved one transitions through the end of life.

I am a dedicated professional with a proven track record as a nurse practitioner in geriatric and palliative care, having served in both acute inpatient and post-acute and long-term care settings.

My extensive experience spans over a decade, during which I have provided comprehensive care to older adults with five or more chronic conditions, often managing complex medication regimens in various residential care environments.
My philosophy revolves around shedding light on fears and beliefs deeply rooted in conventional wisdom, differentiating them from fears and beliefs stemming from negative self-talk and external control over your truth and power. Negative self-talk often tells you that you are unworthy, lack value, are not enough, or are solely responsible for a loved one's complex physical and emotional needs. Once you can distinguish between these fears and truths, you can begin the transformation into an empowered caregiver who can make decisions based on love rather than fear. Love and fear do not coexist harmoniously in the long run.
My core focus is to nurture compassion and authenticity in my connections with families navigating the challenges of end-of-life care. My ultimate goal is to guide them in tapping into their inner reservoir of wisdom and resilience, allowing them to discover a deeper understanding of themselves and engage in activities that bring them fulfillment.
It is a privilege and a source of immense satisfaction to support and witness the growth and well-being of those entrusted to my care.
